leader at the top of this form (this will be the person co-ordinating the sponsorship, with

whom we will be in contact). List overleaf the other members' details, or details of the

you will find on the Child Sponsorship page. If you are a UK taxpayer and are willing for

us to reclaim tax from the Inland Revenue under the Gift Aid scheme, please fill out the

Gift Aid form, also on the Child Sponsorship page.
